When enquiry form is associated with a campaign activity, automatically assign matching contact id (email mathc) to campaign activity as target contact

I have created a form where customers can register their interest in recieving a new catalogue on release - it would be helpful if this automatically associated with their contact ID and the campaign activity. Would also be useful if contacts wanted to sign up to hear about a specific event / product launch that wouldn't fall under normal Mailchimp activities
